This guideline outlines a robust approach to proactive identification of frailty, and frailty syndromes, and the provision of early access to comprehensive geriatric assessment (CGA) and other interventions to reduce harms and improved outcomes. Whilst the focus in mainly on frailty assessment, there is also signposting to guidance applicable to frailty syndromes and the hospital patient journey.
