For patient transport enquiries to attend a hospital appointment, you will be asked a series of questions to meet the eligibility criteria in order to use the service.
Patients are referred by their GP via the Oldham GP referral gateway service for Primary and Secondary care services. Patients will receive a notification from their GP Practice informing them to make contact with the Appointment Centre after 5 working days to book their appointment.
The department has close links with other Northern Care Alliance NHS FT hospital sites, Oldham Community Therapy Services, MSK, Lancaster House, Virgin Care Dermatology, Northwest Ambulance Service and GP Practices in the Oldham area.
