Children’s Community Nursing - Heywood, Middleton, Rochdale (HMR) Community

The children’s community team are available every day from 8:00am to 8:00pm including bank holidays. The teams provide specialist care and treatment to children with a diagnosed acute, chronic and complex condition or illness, aiming to reduce inpatient admissions and prevent avoidable admissions for children and young people.

The teams provide nursing care and advice as appropriate, for neonates, children and young people and families.

Along with the children’s community nurses we have specialist nurses in diabetes, epilepsy and palliative care.

Referral Criteria

Children and young people under 19 years of age
Children who live in Heywood, Middleton and Rochdale (or be registered with a GP in Heywood, Middleton and Rochdale)

Children and Young people who have been seen by a professional and have a diagnosis:

Acute respiratory disorders
Gastroenteritis
Eczema
Constipation

Wound care
Home IV therapy
Enteral feeding
Home oxygen therapy
Tracheostomy care

Long term ventilation
Palliative/ end of life care
Diabetes
Epilepsy

This list is not exhaustive and if you have a patient who you feel our nurses could support but the condition is not listed please contact the team to discuss.

For acute illnesses the team offers an urgent response and we aim to triage all acute referrals within two hours of receiving the referral and will offer same day home visits to for referrals received up to 6pm.

Once our team has received a referral one of our nurses will contact the young person / carer via telephone to arrange support.

If assessment is required we will arrange an appointment to see the child in one of our clinics across the borough or at a home visit.

We will offer follow up support as deemed necessary and young people and their families can contact the team to discuss care and seek advice as required.

If a young person or child is coming home from hospital one of our nurses may be involved in the discharge planning meetings to ensure the family and child have all the equipment and support that you require when your child is discharged home.

The team will refer on to other services as required and may work alongside other integrated children’s services as required or other agencies such as children’s hospice, nurseries and schools to ensure the child, young person and their family get the care and support they require.

Once the child or young person is ready for discharge from the CCNT the team will update the family and the registered GP for the child.

The discharge from children’s community nursing may include transition to adult services and our nurses will aim to co-ordinate this to ensure the process is as smooth as possible.
