Specialist Palliative Care - Bury Community

The Bury Specialist Palliative Care Team are a team of highly skilled clinicians, providing specialist palliative care. We offer high quality person centred care, this includes a holistic approach using advanced communication skills.

We recognise and manage specialist and complex needs, we offer advice and guidance and advanced care planning.

We act as a specialist resource working alongside and in support of generalist services such as GPs and district nurses in the management of symptoms and the provision of psychological and spiritual support to patients and their families.

Care is provided to adults from diagnosis to end of life, where the patient's needs are not being met by the generic Primary Health Care Team (PHCT).  

Care is offered to patients in their own homes, nursing or residential homes.

The service aims are:  To work with PHCT members to help meet the physical, psychological, spiritual and social needs of adult patients and carers experiencing difficulties managing symptoms or adjusting to life with a diagnosis of cancer or other life limiting illnesses. 

To help to facilitate continuity of care across all health care settings so as to enable patients to be cared for in their preferred place of care. 

To work with other members of the specialist palliative care MDT to ensure that the best possible evidence-based care is given to patients and carers.
