Oldham Community Stroke Team is a close-knit team from a variety of healthcare professions.
We generally see individuals with a recent diagnosis of stroke but we can provide limited specialist input to known stroke survivors where appropriate.
The team includes representatives from: Nutrition and Dietetics, Occupational Therapy, Physiotherapy, Specialist Stroke Nursing, Psychology, Speech and Language Therapy and Therapy Assistants.
We also work closely with other organisations including Stroke Consultants and hospital teams, Social Services and The Stroke Association.
Our vision is to provide a comprehensive and high quality service to support stroke survivors and their family members or carers, in a timeley and flexible way.
We aim to promote overall wellbeing and quality of life after stroke and to empower stroke survivors to reach their full potential.
We provide support, advice and rehabilitation as appropriate, to help stroke survivors achieve their personal goals as they adapt to the challenges of life after a stroke.
