Discharge home

District Nurse - if you have a surgical wound, closed with sutures or clips, you will be sent home with a referral to the district nurses.  This will be sent by the ward staff via email or fax, and you should expect a phone call to arrange a visit or clinic appointment to receive wound care.

Stoma Nurse - if you have a stoma, our stoma team will refer you to the community stoma team.  Again, you should expect a phone call within the first few days after discharge to arrange a home visit to assess your stoma and arrange deliveries of your stoma supplies.

Colorectal Nurse / Enhanced Recovery Nurse – once you are discharged from hospital, you will receive a number of telephone follow up appointments from either a Colorectal Nurse Specialist or Colorectal Enhanced Recovery Nurse.  This will give you an opportunity to discuss any concerns and potentially prevent any need for readmission into hospital.
