Diabetes Service - Integrated Community Diabetes Service

Who are we?

The Integrated Community Diabetes Service is a team consisting of Diabetes Consultants, GPSI’s, Specialist Nurses, Specialist Dietitians, a Dietetic Assistant and secretarial support. The service has been commissioned by HMR and Bury CCG to provide specialist diabetes care for appropriate adults with a HMR or Bury GP.

Clinic Locations

We are based at Moorgate Primary Centre in Bury and The Phoenix Centre in Heywood. We run virtual and face to face clinics at a variety of locations across Bury and HMR. Home visits can also be carried out on patients deemed appropriate for this.

How to refer?

You will have been referred into the service by your GP or the hospital diabetes team. If you have been referred by your GP you will be contacted by the appointments centre to choose the most appropriate appointment time and location.

Who will I see?

You will have been triaged to see the most appropriate clinician in the team following receipt of your referral. However, you may also see other team members as appropriate to support your diabetes care.

At your appointment

Please arrive on time as if you arrive more than 10 minutes late we may be unable to see you. Please bring with you:

  • A list of your current medication
  • Blood Glucose (sugar) results if you test at home

Your initial appointment should last approximately 60 minutes. During this time you will be given advice regarding your diabetes with a view to helping improve your overall control. You will be involved in any decisions around changes to your diabetes treatment and an agreed treatment plan discussed.

Follow up appointments will be arranged as appropriate. The aim of the service is to work with you and your GP to achieve improved control and then to discharge you back to the care of your GP for follow up. Following consultations where changes have been agreed, a copy of the management plan will be sent to your GP and a copy to you.

The Integrated Community Diabetes Service will not replace the care you receive from your own GP or practice nurse so ensure that you still attend your annual diabetes checks.

If you fail to attend appointments you will be discharged back to your GP.
