The Frailty, Hospital at Home and Virtual Ward Team are based at St James House, Salford. The service hours are available between 8am - 10pm every day.
You can contact us on 0161 206 6666. You can leave a message when the office is closed if it is not an urgent matter or an emergency.
If your health worsens outside these hours, please contact the out of hours district nursing team on 0161 206 8921 or 999 in an emergency.
Who are We?
We are a team supporting people in the Salford community that allows you to receive hospital care, in the comfort of your own home with support from your consultant.
The team can prevent you going to hospital and/or help support you when leaving hospital, we will aim to get you home from hospital as quickly as possible when it is safe to do so.
Our aim is to monitor and treat your illness to help you recover at home, while promoting your independence with support of family/carer should this be your wishes.
The Team includes:
Advanced Practitioners - diagnose a new health problem and complete a medical management plan to help you recover. Monitor and review conditions and medication. Speaking with other health care professionals
Nurses - help with medication, skin, and wound care, monitor, and support you, giving healthcare advice.
Pharmacist - Advice on medication and how this may help your health condition.
Occupational Therapist - Help you find different ways for doing everyday living tasks sometimes using equipment or alternative methods.
Physiotherapist - Provide exercise programmes targeting your specific needs with the aim to improve your quality of life.
Assistant Practitioners - Help you complete exercise programmes and practise daily activities sometimes using equipment.
Support Workers - Help or support with all aspects of your daily living like personal care, diet, fluids, and medication.
Social workers - Help with the wider aspects of your life including support if longer term care is required.
We also have students and other people completing training who may be involved in your assessment and treatment.
How to Access the Service
Referrals are only received from a Health Care Professional for example, doctors, in the community and hospital. They will discuss this with you and your family/carer should you wish before they refer to hospital at home.
What to Expect
When referred, a nurse will contact the doctor who referred you to hospital at home. The nurse will ask questions to make sure we know what the required treatment is specific with your needs, we will then plan an assessment.
Assessment
Either one or two people will attend your home on the same day to complete an assessment. They will discuss a plan with you and/or your family/friends and how we will support you at home.
This may include visits from different members of the team who will continue to monitor and support you at home.
Please be honest with how you feel when you are asked the questions and should you wish, please involve your family and carer in the discussion. This will help to make sure you receive treatment that is right for you.
We can also provide electronic equipment that will monitor your health condition at home without the team attending, the team member will assess, and discuss this with you. If the equipment if right for you, we will demonstrate how to use it and train you or your family/carers to monitor some things like your blood pressure, temperature via the equipment.
Safety and Security
All our Team wears uniforms and will carry photographic identification badges. Please check this before allowing someone to enter your home.
Discharge
We will plan and discuss with you your discharge from the team. This can be either by telephone or visiting you at home.
When we discharge you, we may arrange different teams to attend and support you at home for a longer period, for example district nurses and care agencies.
Any equipment will be collected from you on discharge.
We will also send your doctor a letter telling them about your episode of care with the team.
Feedback
You may be asked to give some feedback about you experience to help improve services. A family member or carer can assist you with this feedback.
Sharing information
To improve the quality of your care and with your consent, information is shared securely between health and care organisations involved in your care. Please let a member of staff know if you do not wish your information to be shared in this way.
