We are a team of Health Professionals that may include Advanced Practitioners Nurses, Occupational Therapists, Physiotherapists and assistants, wanting to help and support you in your own home.

The aim of our service is to keep you safe at home, we can support you with a health care related need that requires urgent attention. This might include infections, feeling generally unwell, reduced mobility, needing equipment, and falls. We will arrange onward referrals to relevant teams that might be able to help you longer term.

We work closely with other colleagues across Oldham to help with ensuring you receive the best and most appropriate care for your current needs, based on an individual assessment.

Our contact number is 0161 770 6771 and we are open 8am – 8pm, 7 days a week, 365 days a year.

Some help we may advise to support you staying at home can include:

• Clinical assessment in your own home
• Monitoring vital signs - we may ask for your help with this
• Therapy assessments
• Equipment provision
• Short stay in intermediate care setting
• Joint assessment and care planning with other health professionals
