About HMR Rapid Response Team
We are a multidisciplinary team who work in the patient’s own home to provide prompt assessment and interventions at times of crisis for those who can be safely cared for at home instead of being admitted to hospital.
The aim of the service is to prevent unnecessary hospital admissions by providing holistic support at home to improve health, independence and quality of life.
What services can we offer?
We can provide assessment within as little as 2 hours.
We can support with health, mobility and personal care needs, by providing advanced clinical examinations and treatments, therapy assessments and treatments, essential equipment supply and provision of short-term carers.
We can provide advice over the telephone, or face to face assessments and intervention, then refer on to other appropriate community services.
We can also link with GP surgeries, other healthcare professionals, social services and refer directly to hospital wards and intermediate care rehab units to support and streamline care.
The Multi-Disciplinary Team
Our multidisciplinary team includes advanced practitioners, nurses, physiotherapists, occupational therapists, therapy assistants, pharmacists and administrative support.
Who can access the service?
Our service is available for patients experiencing an acute physical health crisis, aged 18 and over, who live in and/ or are registered with a GP in the Heywood, Middleton and Rochdale area. Our service is for patients who would otherwise require an acute admission to hospital.
How to refer to the service
We accept referrals from all health and social care professionals, such as GPs, nurses, therapists, ambulance service, care homes, A&E and social care workers. We are also happy to accept self- referrals from patients or carers.
