What is Intermediate Care?
Intermediate Care are community, short term rehabilitation beds which support you to work with a multi-disciplinary team to gain as much independence as possible and help you return home.
What is Medlock Court?
Medlock Court is a residential bed-based unit providing support from a range of people, including therapists, to help you to achieve your goals.
Your stay at Medlock Court
Medlock Court is a residential enablement service, providing support for a short time (no longer than 6 weeks) to help you to rehabilitate and increase your independence.
This support is provided by a team of people including care staff, physiotherapists, occupational therapists and assistants, social workers, pharmacist technicians, district nurses and doctors.
Enablement Service may help you:
- Rehabilitate after a fall, an acute illness or an operation
- Avoid going into hospital unnecessarily
- Return home more quickly after a hospital stay
How is enablement different to other health and social care support?
- Enablement is a non-chargeable, short-term service
- You will receive intensive support from a range of professionals
- You'll work with staff to agree your goals (for example making a meal, dressing independently and walking safely) and how to achieve them
- Care staff will support you to practice tasks on your own
Stages of Enablement care - what to expect
At the start
- You will be given an initial assessment to determine your current ability and falls risk
- You'll be given information about the service and what will be involved
- You will be involved in the creation of a support plan which will include what you are aiming for (your goals) and how to achieve them. These discussions can include your family and carers if you wish
- You'll have the opportunity to ask questions
While you are receiving the service
- You'll receive support from a range of people, including therapists, to help you to work towards your goals
- You’ll participate in washing and dressing assessments and kitchen assessments alongside a member of the therapy team, in addition to ongoing practice with the care staff. You’ll be encouraged to complete tasks as independently as possible
- You may require a home access visit by an occupational therapist to assess your home environment and to determine any need for equipment or minor adaptations
- You’ll receive a personalised exercise programme
- You will be enrolled into group sessions to suit your needs
- Your length of stay will be determined by the progress you make
At the end of the Enablement Service
- If you need ongoing support, you'll be given a plan for transferring to another service such as community physiotherapy, community occupational therapy, Age UK or a social prescription
- You may require a social work referral for care or reablement support on your discharge
- Any pre-existing care and support will be restarted or increased as needed
- Information about other types of support available will be provided
Family and Friends are welcome to visit
Visiting times are:
11am - 12 noon
1.30pm - 4.30pm
5.30pm - 7.30pm
