Introduction
The Community Neuro Rehab Team (CNRT) is a multidisciplinary team comprising of physiotherapists, occupational therapists, neuro rehab nurse and therapy assistants who specialise in neurological conditions.
Roles within the team
Physiotherapy
Assess and identify any physical impairments, then implement an appropriate programme of physical therapy in order to improve function.
Neuro rehab nurse
To provide support and advice regarding the management of your condition. They can also liaise with other members of the team as required.
Occupational Therapy
Assess and support the relearning of daily living skills. They can complete an assessment of your environment and can organise equipment for your home if required. Assess and provide cognitive rehabilitation.
Therapy assistants
Support the therapists to deliver your rehabilitation programme.
Parkinson’s nurse
The Parkinson's Disease Nurse Specialist (PDNS) works as part of the team providing support for people with Parkinson's disease and related conditions.
Team admin
First point of contact when you call the team and will pass your messages onto the relevant team members. Working behind the scenes to support your rehabilitation.
What to expect from the Therapy Team
The team provides interventions for adults for those who's primary condition is neurological, other than stroke, who are registered with an Oldham GP.
The team works closely with other health, social and vocational services including: the neuro rehabilitation consultants, speech therapy, psychology, orthotics and dietetics teams. The service is generally delivered at the patient's home, but some patients are also seen in clinic.
Following a diagnosis of a neurological condition, you will be assessed to identify your rehabilitation need and an individual treatment plan agreed, with the aim of helping you to achieve the maximum potential possible.
Therapists work together with patients and their family/support network to set realistic goals and provide treatment sessions to suit individual needs.
Therapy input is time limited based upon your progress and goals will be regularly reviewed.
Once specialist neuro-rehabilitation is no longer required, we can support patients to self-manage and access other services if appropriate.
Self-management means empowering you to take control of your condition either independently or with the support of carers.
Once discharged, patients can be referred back if there is a significant change or if new goals are identified.
What to expect from the Parkinson’s nurse
The Parkinson's service has an open referral system and receives referrals from patients, carers or health care professionals.
The Parkinson’s nurse provides on-going assessment, advice on symptom control, medicine management, education, liaison and onward referral for patients and their family/carers.
The nurse works closely with the specialist consultant physicians and neurologists at the Royal Oldham Hospital and care is delivered to meet patients' needs and preferences in clinic or in their own home.
