About our service

The Children’s Community Nursing (CCN) Team provides specialised nursing care for children and young people aged 0-18 years with acute, long-term and complex health needs, in order to reduce or prevent hospital attendance or admission.

The care that we offer on the Children’s Community Team includes (this list is not exhaustive):

●    Nursing care during both short and long term illnesses, including administering intravenous antibiotics in a child’s home
●    Acute asthma care and management
●    Children undergoing cancer treatment, including support and chemotherapy administration
●    Provide families with palliative and end of life care support
●    Children who require long term ventilation at home
●    Equipment provision and training in a child’s home, school and nurseries
●    Wound care, burns/scalds and suture removal

We can also provide advice and support to children and young people virtually and face to face in their home environment and in local clinics. Additionally, some children may be seen in schools or nursery settings.

Referrals to our Service

Referrals to the Children’s Community Team are accepted from any professionals working with the child or young person, providing a clinical diagnosis has been made. Referrals can be sent via our secure email address.

Nurse-led Clinics

We offer nurse-led clinics within a variety of Salford community gateways. The clinics operate an appointment system which can be booked via our office telephone number 0161 206 2370. If, however, this appointment becomes inconvenient, please ring our office telephone number to arrange another appointment time.

All our staff that come into contact with children have a responsibility to safeguard and promote their welfare. Everything we discuss will be confidential. However, if we have concerns or are worried by anything we observe, we have a responsibility to contact the children’s safeguarding/social care team, providing it doesn’t place the child in any risk. Any referral will be discussed with parents/guardians.

Code of Conduct and acceptable behaviours

We ask parents or guardians to refrain from smoking prior and during a home visit.

The Children’s Community Nursing service will not accept patients, carers or relatives verbally or physically assaulting any of their staff.  Any incidents will be investigated and may result in being reported to the police.

Please be aware that there will be occasions when our nurses are accompanied by students. If you do not wish for them to be in attendance, please inform a team member.

If a nurse is not available when you telephone, please leave a message on the answer phone and a nurse will attempt to call you later that day.

Please ensure you leave your name, your child’s name, address, contact telephone number and the reason why we are involved with your child’s care.

Please note: non urgent messages may not be dealt with the same day (If you are concerned about your child out of office hours, please call 111 - if you think your child requires an ambulance call 999).
