The Salford Urgent Response Team is a based at St James House, Salford.
The Salford Urgent Response Team are a community based multi- disciplinary team (MDT) within the Northern Care Alliance.
Refer your patients on 0161 206 6666 daily between 8am - 8pm.
Our crisis response to those experiencing an acute episode of illness or injury contributes to reducing Emergency Department (ED) attendance and unnecessary hospital admission. We can also expedite hospital discharge from wards for patients requiring further medical input in their normal home environment.
Wherever possible, the patient will be maintained in their own home through a holistic multidisciplinary approach including:
- Advance Clinical Practitioners
- Non-Medical Prescribers
- Nurses and Nursing Associates
- Occupational amd Physiotherapists
- Assistant Practitioners
- Community Support Workers
- Administrators
- Pharmacist
- Social Worker
- Access to out of hours nursing team, district nursing, rehabilitation teams and residential rehabilitation beds
The Urgent Response Team can
- Make the person/situation safe
- Provide therapy input and equipment
- Review clinical records and historical diagnostic tests
- Monitor patients for deterioration
- Obtain and review blood, urine, and other diagnostics
- Obtain holistic history, assessment, diagnose and treat patients
- Prescribe appropriate medications for acute presentations
- Medicines optimisation assessment
- Administer medication to resolve an immediate crisis, including nebuliser and intravenous therapy
Typical referrals can Include:
Any patient at risk of attending ED or hospital admission:
- New/acute problem (e.g., infection) including new confusion and delirium
- An exacerbation of a chronic condition
- Falls with no apparent serious injury
- Decompensation of frailty
- Deconditioning/reduced mobility
- Urgent equipment to support those experiencing a crisis
- Urgent existing catheter care
- Urgent support for diabetes
- Serious illness where treatment at home is in keeping with the person’s declared or recorded wishes (e.g., Advanced Care Plan)
- A patient in receipt of palliative care who may be in an acute crisis
- A breakdown of unpaid carer arrangements
In addition, the Urgent Response Team also have specific pathways that provides.
- Home assessment and IV treatment of:
○ Pneumonia
○ Pyelonephritis
○ Cellulitis
○ Bronchiectasis (Diagnosed) - Assessment and management of:
○ DVT (lower limb only) - Monitoring of:
○ Stable heart failure - Falls prevention
- NB: Additional exclusions for IV and DVT apply and will be checked on the telephone referral
Service Inclusion Criteria:
- At least 18 years old
- A resident of Salford AND
- Registered with a Salford GP service AND
- Currently physically located within Salford
Service Exclusion Criteria:
- Under 18 years old on day of referral
- Not resident of Salford
- Not registered with a Salford GP
- Not currently physically located within Salford
- Any acute mental health presentation
- Any acute presentation related to, or presenting with current intoxication or withdrawing from alcohol and/or drugs (illicit or non-illicit)
- Any acute presentation related to a pregnancy, gynaecology, or obstetric problem
- Current aggressive behaviour putting attending staff at risk
- Any dental problem
