Intermediate Care - Welcome to Wolstenholme Intermediate Care Unit

About the Unit

Wolstenholme IMC Unit is to provide you with a opportunity for rehabilitation and re-ablement with the support of a wide range of health care professionals.

The unit is also resourced to accept patients with continuing illness recovery for example you may be recovering from a chest infection and require treatment for this.

The unit has a team of nurses available 24 hours a day 7 days a week. The medical cover is provided by general practitioners Monday to Friday 9am - 5pm.

We have full time physiotherapists and occupational therapists who will support you in regaining your mobility and function.

The team will identify areas which you may be struggling with currently and together we will work with you to regain your confidence and the skills required to live an independent and fulfilling life.

We will encourage you to be involved in all aspects of your re- ablement and care. This can include washing and dressing, using the toilet, moving on and off the bed or chairs safely, and preparing simple drinks and meals.

There is a dedicated pharmacist and pharmacy technician who will work closely with the medical staff to ensure you are receiving the right medication. They are on the unit Monday to Friday 8am - 12 noon.

We have an activity co-ordinator whose aim is to provide opportunities for social interaction and engagement and offer activates such as baking, crafts, discussions, reminiscence and much more.

On warmer days we have an outside terrace with outside garden furniture, we also have gardening club which takes place on the terrace. Please feel free to ask staff to open the terrace for you to sit outside with your family/friends.

We also have a hairdresser who attends once a week, service users can access this although there is a charge, a price list is available upon request and money can be brought in on the day if necessary.

Meal times

The ward has open visiting. However, we operate a protected meal time policy. This is to ensure that you have an uninterrupted time when you can eat your meals in peace and that nursing staff are on hand to help if required.

Visitors are asked not to visit during these times unless they are there to help patients to eat. Mealtimes are at:

  • Breakfast: 9am
  • Lunch: 12noon
  • Tea: 5pm

We take nutrition very seriously. It is a paramount to recovery and is a top priority for all our staff involved in your care.

Meal times are encouraged to take place in the dining area, which provides you with an opportunity to meet other services users. A member of the therapy team may suggest you would benefit from joining the breakfast club, where you will be supported to prepare your own breakfast choices.

Feedback is of high importance to us and we value the opportunity to hear your thoughts regarding your stay! To help us improve our service we encourage patients and carers to complete a ‘Friends & Family Test’ survey when you leave our ward by scanning the QR code below. The comments our patients give get reviewed on a monthly basis and are a really valuable way of getting feedback on how we are doing and what we could improve on.
